About The Position

Amedisys is hiring a Medical Social Worker to provide social work services to patients receiving care at home. The role supports patients and families with health-related financial, social, and emotional concerns, helps them develop effective coping skills, and provides services in accordance with the plan of care. The Medical Social Worker coordinates with physicians, healthcare team members, and care center staff, educates patients and families about interventions and treatment plans, and contributes to care planning, transfers, and discharges. The position requires an Oregon LMSW or LCSW license and pays $45.00 to $65.00 per hour, with compensation varying by geographic market, job-related knowledge, skills, and experience.

Requirements

• Must hold an Oregon LMSW or LCSW license.
• Master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
• One year of social work experience in a healthcare setting.
• Meet state and federal requirements for the position.
• Current CPR certification.

Responsibilities

• Assist patients and families with health-related financial, social, and emotional concerns and effective coping skills.
• Provide social work services in accordance with the plan of care.
• Communicate changes in a patient's clinical status to healthcare team members and care center office staff in a timely manner.
• Educate patients, family members, and other team members on medical social work interventions and the treatment plan.
• Perform care planning, transfer, and discharge functions.
• Identify risks or threats to a patient's health in their environment and promptly implement appropriate interventions to reduce them.
• Communicate identified risks or threats to the physician and other healthcare team members to coordinate care effectively.
• Perform other duties as assigned.
