About The Position

VNA Alliance is hiring a full-time Medical Social Worker to provide medical social services as part of an interdisciplinary team. The role delivers rehabilitative and supportive casework to help patients achieve optimal social and health adjustment, while helping patients and families understand and follow medical recommendations. The Medical Social Worker collaborates with physicians, patients, families, and team members to develop and adjust care plans, connects patients with community resources, and addresses personal or environmental challenges that may limit access to medical care. The position also supervises the treatment program of the social worker assistant and reviews and co-signs clinical visit notes as appropriate. The position has a flexible schedule and requires reliable transportation for home visits.

Requirements

• Master’s degree from a school of social work approved by the Council on Social Work Education.
• State licensure recommended.
• One year of social work experience in a healthcare setting.
• Reliable transportation for home visits, with a valid driver’s license and insurance as per personnel policies.

Responsibilities

• Provide rehabilitative and supportive casework to help patients achieve optimal social and health adjustment.
• Assist patients and families in understanding and following medical recommendations.
• Develop care plans collaboratively with physicians, patients, families, and team members, adjusting them as needed based on patient conditions.
• Facilitate access to community resources, acting as an intermediary when necessary.
• Address personal and environmental challenges that hinder patients from maximizing their medical care benefits, including counseling and assisting with admissions to care facilities.
• Supervise the treatment program of the social worker assistant, reviewing and co-signing clinical visit notes as appropriate.
