About The Position
Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to provide home health services on a PRN/per diem basis. The position offers flexible scheduling, the ability to accept visits based on availability, and competitive per-visit compensation; hourly compensation may also be available depending on the assignment. Additional assignments may become available as patient census and coverage needs grow. This is a field-based role requiring travel to patients’ homes throughout the assigned service area. Candidates must have a Master’s Degree in Social Work, a full, active, unrestricted social work license, a valid driver’s license, a reliable personal vehicle, and current auto insurance with the candidate’s name listed on the policy. Limited, temporary, associate-level, provisional, and other restricted licenses are not accepted.
Requirements
- Master’s Degree in Social Work (MSW).
- Full, active, unrestricted social work license; limited, temporary, associate-level, provisional, or otherwise restricted licenses are not accepted.
- Valid driver’s license.
- Reliable personal vehicle.
- Current auto insurance with the candidate’s name listed on the policy.
- Strong knowledge of community resources.
- Excellent communication and documentation skills.
- Ability to work independently in patients’ homes.
- Reliable, compassionate, and professional.

Responsibilities
- Complete psychosocial assessments.
- Evaluate patient and family needs.
- Provide counseling and emotional support.
- Connect patients with community resources and support programs.
- Assist patients with financial, transportation, housing, and long-term care resources.
- Support patients and families with adjustment to illness or disability.
- Participate in interdisciplinary care planning.
- Coordinate with nurses, therapists, physicians, and other clinical staff.
- Document visits and interventions accurately and promptly.
- Travel to patients’ homes throughout the assigned service area.
