About The Position

Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) for its home health team on a PRN/per diem basis. This field-based position involves travel to patients’ homes throughout the assigned service area. It offers flexible scheduling, the ability to accept visits based on availability, high per-visit compensation, and potentially hourly compensation depending on the assignment. Additional assignments may become available as patient census and coverage needs grow. The position is intended for supplemental work and does not involve a full-time commitment.

Requirements

• Master’s degree in Social Work (MSW).
• Full, active, unrestricted social work license; limited, temporary, associate-level, provisional, and restricted licenses are not accepted.
• Valid driver’s license.
• Reliable personal vehicle.
• Current auto insurance with the applicant’s name listed on the policy.
• Strong knowledge of community resources.
• Excellent communication and documentation skills.
• Ability to work independently in patients’ homes.
• Reliable, compassionate, and professional.

Nice To Haves

• Home health, hospice, hospital, rehabilitation, or community-based experience.

Responsibilities

• Complete psychosocial assessments.
• Evaluate patient and family needs.
• Provide counseling and emotional support.
• Connect patients with community resources and support programs.
• Assist patients with financial, transportation, housing, and long-term care resources.
• Support patients and families as they adjust to illness or disability.
• Participate in interdisciplinary care planning.
• Coordinate with nurses, therapists, physicians, and other clinical staff.
• Document visits and interventions accurately and in a timely manner.
• Travel to patients’ homes throughout the assigned service area.
