This role involves completing psychosocial assessments for patients' post-hospital care, assisting them in coping with stress related to hospitalization and illness, and collaborating with the Clinical Care Management team to determine appropriate care settings. The Medical Social Worker will also serve as a resource to hospital staff and physicians regarding the psychosocial aspects of patient care, provide information and education on community resources for post-hospital care, and make necessary referrals. Additionally, the role includes reporting suspected cases of abuse/neglect/exploitation and acting as a liaison between the hospital, patients, and the Department of Social Services. Consultative services may be requested outside of normal working hours.
