Requirements

Current Medical Social Worker (MSW) license in the state of employment (limited licenses not accepted)
Master's Degree in Social Work (MSW) required
Minimum of one year of MSW experience in an acute care setting
Valid driver's license and current auto insurance
Ability to meet the physical demands of the position

Responsibilities

Complete comprehensive patient assessments.
Participate in developing individualized Plans of Care.
Identify barriers to care and recommend appropriate interventions.
Connect patients and families with valuable community resources.
Assist with financial assistance applications and long-term planning.
Educate and support patients and caregivers throughout their care journey.
Provide counseling related to adjustment, coping, loss, and grief.
Collaborate with physicians, nurses, therapists, and the interdisciplinary care team.
Complete accurate clinical documentation in accordance with agency, Medicare, and federal guidelines.
Observe, document, and communicate changes in patient condition.
Attend required training and agency meetings.
