Hospice Medical Social Worker (MSW)
About The Position
The Hospice Medical Social Worker (MSW) evaluates and develops a plan of care personalized to fit the patient’s psychosocial needs. This role assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment, and communicates findings to the interdisciplinary care team. The MSW counsels patients and families/caregivers as needed regarding stress and other identified coping difficulties, and provides crisis intervention when necessary. They also educate family/caregivers on and assist in the preparation of advanced directives, and help plan for funeral arrangements, financial, legal, and healthcare decision responsibilities. The role assesses the patient’s medical and nursing/therapy needs in the home setting, financial resources, and available community resources. The MSW observes, documents, and reports changes in the patient’s condition to the interdisciplinary care team and Clinical Manager. They actively participate in quality assessment performance improvement (QAPI) teams and activities. Additionally, the MSW is able to supervise/instruct the performance of the Social Work Assistant (BSW/LSW) to ensure all medical social services are provided in compliance with company, government, and professional standards. The role requires maintaining and submitting documentation timely per company policy, including case conferences, patient/physician community contacts, visit reports, and progress notes.
Requirements
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Social Worker licensure in the state of practice; if required by state law/regulation
- Valid driver’s license and reliable transportation
- Proof of automobile insurance (MVR checked)
- Proof of current CPR certification
- Able to pass criminal background check and national sex offender clearance

- Assists family and patient in planning for funeral arrangements, financial, legal and health care decision responsibilities.
- Assesses any relationships of the patient’s medical and nursing/therapy needs in the home setting, financial resources and available community resources.
- Observes, documents and reports changes in the patient’s condition to the interdisciplinary care team and Clinical Manager.
- Actively participates in quality assessment performance improvement (QAPI) teams and activities.
- Able to supervise/instruct the performance of the Social Work Assistant (BSW/LSW) to assure that all medical social services are provided to patients in compliance with company, government and professional standards as applicable.
