Requirements

Master's Degree in Social Work (MSW) from an accredited program.
Valid Arizona driver's license, reliable transportation, and current auto insurance.

Nice To Haves

Previous hospice, home health, palliative care, or healthcare experience preferred.
Strong communication, counseling, and interpersonal skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.

Responsibilities

Provide psychosocial assessments for hospice patients and their families.
Develop individualized care plans that address emotional, social, financial, and community resource needs.
Offer counseling, crisis intervention, grief support, and emotional guidance to patients and loved ones.
Assist families with advance care planning and end-of-life decision-making.
Connect patients and caregivers with community resources and financial assistance programs.
Collaborate closely with physicians, nurses, chaplains, aides, and the interdisciplinary hospice team.
Maintain accurate, timely documentation in accordance with agency, state, and federal regulations.
Participate in care conferences and contribute to high-quality patient-centered care.
