About The Position

We are hiring a Medical Social Worker (MSW) for a Hospice position providing care to patients in their homes in the Kansas City, MO. area. This role involves delivering compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life. The position requires conducting comprehensive psychosocial and bereavement assessments, identifying patient and family needs, and developing care plans that support emotional, social, and end-of-life goals. It also involves providing counseling, emotional support, and guidance to patients and families, connecting them with resources and helping them navigate the end-of-life journey. The role requires critical thinking and compassionate problem-solving to address complex situations, advocating for patient wishes, and ensuring accurate, timely, and compliant EMR documentation. The position is designed for success with a dedicated field education team, supportive leadership, and a collaborative culture. The role requires travel to patient homes within an assigned territory.

Requirements

• Master’s in Social Work from graduate school accredited by the Council of Social Work
• One (1) to two (2) years post graduate social work experience in a health care setting
• Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Apply critical thinking and compassionate problem-solving to address complex situations, advocate for patient wishes, and ensure accurate, timely, and compliant EMR documentation
• Work independently but never alone
• Travel to patient homes within assigned territory
