OSU Physicians is seeking a Licensed Social Worker (LSW) to support its growing clinical services within the Internal Medicine team. Requirements

  • Minimum of a Bachelor’s Degree in Social Work or a Master Degree preferred.
  • State of Ohio licensure (LSW)
  • Experience in a healthcare environment.
  • Experience coordinating available community resources and payer programs and limitations is required.
  • Flexible to meet the needs of the patients and providers.
  • Demonstrative effectiveness in conveying information both in writing and verbally.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Position continually requires demonstrated poise, tact and diplomacy.

Responsibilities

  • Work with physician, staff and other health care team members to identify at risk populations (i.e., financial hardships, lack of transportation, abusive situations and malnutrition) and consult as necessary.
  • Assess needs of identified patients and provide case management.
  • Recommend interventions and implement a plan to assist the patient.
  • Link patients with systems that will provide needed resources and opportunities.
  • Provide coordination of community resources and maintain liaison with community agencies.
  • Serve as liaison between each patient and staff, as well as community resources.
  • Serve as patient advocate by coordinating services, involving the patient in a case plan.
  • Educate Faculty Physicians, Residents and Staff of the referral process and community resources available to patients.
  • Provide ongoing case management as needed.
  • Provide excellent clinical services.
