About The Position

The Social Worker provides face-to-face psychosocial assessments, counseling, and resource coordination to support patients and families through their healthcare experience. This role involves collaborating with the multidisciplinary team to ensure that patients receive safe, timely, and comprehensive care. The Social Worker advocates for patient needs, assists with discharge planning, and serves as a liaison for child and adult protective services, guardianship, foster care, and community resources. The position requires expertise in crisis intervention, legal mandates, and culturally competent care. Requirements

  • Master’s Degree in Social Work (MSW) required.
  • Permanent LSW licensure required.
  • Certification in Crisis Prevention Intervention or equivalent de-escalation training within 6 months of hire.
  • Knowledge of evidence-based psychosocial assessment, short-term therapeutic counseling, and crisis intervention for mental health and substance use disorders.
  • Understanding of disease processes and human behavior related to health and social needs.
  • Strong discharge planning skills for safe patient transitions.
  • Knowledge of legal mandates for child and adult protective services.
  • Familiarity with growth and development principles across the lifespan, including end-of-life care.
  • Ability to identify decision makers and conduct goals of care conversations.
  • Experience adapting services for diverse populations, including geographically, culturally, and financially varied groups.
  • Ability to develop and maintain collaborative relationships with hospital leadership, law enforcement, legal agencies, schools, probation, and community organizations.
  • Proficiency in electronic medical records and documentation practices.
  • Skilled in crisis intervention and de-escalation techniques.
  • Willingness to work on-call and holiday rotations as needed.

Responsibilities

  • Meet with patients and families to assess psychosocial needs and provide counseling.
  • Collaborate with healthcare team members to facilitate safe, timely, and resource-appropriate treatment episodes.
  • Interpret complex patient and family needs and model caring practices for staff.
  • Evaluate prior functioning, support systems, and coping mechanisms to identify psychosocial risks.
  • Provide counseling on emotional, social, and financial impacts of illness or disability.
  • Access and mobilize family and community resources to meet patient-specific needs.
  • Conduct mental health assessments upon physician referral and support complex discharge planning.
  • Serve as the primary liaison for cases involving abuse, neglect, domestic violence, and protective services, coordinating with legal and community agencies.
  • Provide intervention in guardianship, foster care, adoption, and mental health placements.
  • Offer specialized mental health services, including crisis intervention, addiction assessments, and psychiatric referrals.
  • Support individuals in crisis with de-escalation and violence prevention techniques.
  • Assist with end-of-life care, goals of care conversations, and managing patient throughput effectively.
  • Document all interventions and case management activities accurately in the electronic medical record.
  • Develop and maintain strong working relationships with hospital staff, law enforcement, APS/DCS, schools, probation, and community agencies.
