Requirements

  • Master’s degree in social work
  • NYS Licensed Master Social Worker LMSW Limited Permit - Those who are hired with a limited permit must have their full license 1 year after obtaining their limited permit.

Responsibilities

  • Conducts psychosocial assessments of patients.
  • Develops individualized care plans based on patient needs.
  • Provides counseling and support to patients and families.
  • Coordinates with healthcare teams to ensure comprehensive patient care.
  • Advocates for patients’ needs and rights within the healthcare system.
  • Assists patients in navigating the healthcare system and accessing resources.
  • Contributes to discharge planning and continuity of care for patients.
  • Provides crisis intervention and emergency support.
  • Facilitates guardianship applications and court hearings.
  • Organizes and leads support groups for patients and families.
  • Educates patients and families about healthcare options and resources.
  • Collaborates with community organizations to connect patients with additional services.
  • Participates in multi-disciplinary team meetings to discuss patient care.
  • Assists patients with financial concerns and connects them to relevant resources.
  • Provides bereavement counseling and support.
  • Documents all patient interactions and treatment sessions in compliance with relevant regulations and organization policies.
  • Works collaboratively within a multi-disciplinary treatment team.
  • Adheres to all state and federal regulations surrounding HIPAA/Title 42 requirements and maintains strict confidentiality.
  • Serves as a mandated reported and follows all legal and ethical guidelines.
  • Participates in clinical supervision in accordance with NYSDOE regulations.
  • Maintains licensure throughout entirety of employment.
