About The Position

Andwell Health Partners is seeking a Per Diem Medical Social Worker - LCSW to support Palliative Care, Hospice, and Home Health services. This role involves assessing and identifying clients' holistic health care needs within their family and community context. The Medical Social Worker will evaluate social and emotional factors impacting the patient's illness, care needs, treatment response, and adjustment to care. A plan of care will be developed under physician orders with measurable outcomes, in collaboration with the client/family and healthcare team, adhering to Organization protocols. Patient care plan activities will be implemented to promote independent living for the patient/family. The role also includes serving as a resource for staff education and training and participating in case conferences.

Requirements

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure- LCSW required
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.

Responsibilities

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Assesses social/emotional factors that are related to the patient’s illness, need for care, response to treatment, and adjustment to care.
• Develops a plan of care under physician orders with realistic/measurable outcomes in collaboration with the client/family and health care team providers according to Organization protocols.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
