About The Position

Hinds LifeCare, a well-respected nonprofit agency in California’s Central Valley and a pioneer in hospice and palliative care, is seeking a Full Time, Medical Social Worker / LCSW to join their team. This role, under operational direction, performs the role of social worker as a member of the interdisciplinary team. The Hospice Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families. Requirements

Master's degree (MSW) from a School of Social Work accredited by the Council on Social Work Education
Minimum one (1) year of Medical Social Work experience required
Electronic medical record experience preferred
Ability to learn and assimilate the EHR Computer Program/System
Demonstrated ability to work with health team personnel
Must have a valid drivers license and a willingness to use personal vehicle in the course of employment
Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period)
Ability to meet the physical demands of the job (ie, Push, Pull, Lift/Carry 10 lbs)
