About The Position

Hinds LifeCare, a well-respected nonprofit agency in California’s Central Valley and a pioneer in hospice and palliative care, is seeking a Full Time, Medical Social Worker / LCSW to join their team. This role, under operational direction, performs the role of social worker as a member of the interdisciplinary team. The Hospice Social Worker completes psychosocial assessments and provides ongoing psychosocial support to patients and families. Requirements

  • Master's degree (MSW) from a School of Social Work accredited by the Council on Social Work Education
  • Minimum one (1) year of Medical Social Work experience required
  • Electronic medical record experience preferred
  • Ability to learn and assimilate the EHR Computer Program/System
  • Demonstrated ability to work with health team personnel
  • Must have a valid drivers license and a willingness to use personal vehicle in the course of employment
  • Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36 month period)
  • Ability to meet the physical demands of the job (ie, Push, Pull, Lift/Carry 10 lbs)

Nice To Haves

  • Palliative, Hospice and or Home Health Care experience preferred
  • Bilingual (English/Spanish) a plus but not required
