About The Position

ChristianaCare HomeHealth is seeking a full-time Medical Social Worker to support patients as they transition from acute and post-acute settings to their homes. The role serves Dover, Smyrna, Frederica, and surrounding areas, working with patients, families, physicians, and other healthcare providers to develop plans that support safe transitions home. The position works day shifts, 8:30 a.m. to 5:00 p.m., with occasional weekend or holiday work based on departmental needs.

The Medical Social Worker participates in multidisciplinary care planning, completes clinical social evaluations and home visits, identifies appropriate specialty programs and Telehealth services, and refers patients and families to community resources. The successful candidate discusses evaluation findings with a Senior Medical Social Worker. A tablet and MatrixCare are provided for patient-record documentation.

Requirements

• Master’s degree from a school of social work accredited by the Council on Social Work Education.
• At least 2 years of experience in a healthcare setting.

Nice To Haves

• One year of home care or community field experience preferred.

Responsibilities

• Participate in case conferences and develop total plans of care with the multidisciplinary team.
• Conduct clinical social evaluations and plan appropriate interventions based on evaluation findings.
• Identify patients appropriate for specialty programs and Telehealth.
• Complete home visits with clients and discuss evaluation findings with a Senior Medical Social Worker.
• Identify appropriate community resource referrals to address clients’ and families’ practical and environmental needs.
