About The Position
The Medical Social Worker II provides support and assistance to patients and their families in planning their needs and arranging appropriate services. The role coordinates services, facilitates discharge and access to services, and promotes continuity of care by connecting the Health System with community resources and other organizations to support integrated care.
Requirements
- Master’s degree in social work from an accredited institution.
- Demonstrated knowledge and skill in advanced-level social work practice.
- Ability to work independently, make flexible decisions, and prioritize assignments.
- Ability to develop and maintain collaborative working relationships with team members and community agencies.
- Knowledge of community resources.
- New York State licensure; an MSW with licensure obtained within 12 months is accepted.
Nice To Haves
- LMSW preferred.
- Hospital social work experience preferred.
- Two years of experience in the social work field preferred.
Responsibilities
- Support patients and families in planning their needs and arranging services.
- Coordinate services and facilitate discharge and access to services.
- Promote continuity of service and connect the Health System with community organizations and resources.
- Participate in rounds to support an interdisciplinary approach to hospital care.
- Use social work theory and practice models to guide patient care plans.
- Complete mental health assessments when needed.
- Complete assessments for the Birthing Center as needed.
- Complete safety planning with patients as needed.
- Provide linkages and referrals to community resources as needed.
- Proactively resolve problems and recommend and implement continuous quality improvement.
