Mercy Haven is opening Long Island's first Medical Respite Program in Bay Shore. This 10-bed program will provide short-term, recuperative care for individuals experiencing homelessness or unstable housing who are recovering from or preparing for surgery. The Medical Respite Program Coordinator serves as the primary case management for the Medical Respite program. This role is responsible for coordinating client care and ensuring that individuals experiencing homelessness receive safe, supportive, and medically appropriate services. The Coordinator works closely with medical providers, social service agencies, and community partners to facilitate seamless care transitions, improve client outcomes, and support program operations.
