About The Position

Mount Sinai Medical Center is seeking a detail-oriented HIM Medical Records Clerk to join their team. This entry-level position is responsible for preparing, scanning, and accurately indexing inpatient and outpatient medical records into the electronic health record system. The role ensures the integrity and quality of patient documentation through thorough quality checks, resolving image issues, and maintaining correct document order, types, and encounters. The Medical Records Clerk supports daily workflow through efficient chart preparation, timely scanning, and adherence to productivity and accuracy standards. Key responsibilities include maintaining strict patient confidentiality, promptly communicating issues, and assisting with additional duties such as chart pickup and workflow coverage as needed.

Requirements

  • High School Diploma and some advanced technical training.
  • One year of experience in a hospital medical record setting.
  • Additional experience in quality analysis or similar position preferred.
  • Possess strong organizational, time management and prioritization skills.

Responsibilities

  • Prepares both inpatient and outpatient discharged records to be scanned and indexes into the Horizon Patient Folder (HPF) on a daily basis.
  • Maintains the integrity of patient information by accurate and timely indexing/scanning documents into the electronic health records and performs quality checks to ensure documents are being scanned accurately.
  • Scans all documents through the Bell & Howell scanner into the Optical Imaging System and re-scans documents when necessary to ensure clarity and accuracy.
  • Maintains daily workflow, insuring that records are scanned in a timely and efficient manner to avoid delay in further processing.
  • Verifies the electronic document against the paper document to ensure correctness.
  • Performs indexing process with accuracy by ensuring documents are indexed to the correct patient, correct encounter and document name.
  • Assigns each document to the correct document type and verifies that bar-coded documents are correctly indexed.
  • Verifies the quality of images as readable by comparing the document on the computer terminal to the original to ensure accurate indexing.
  • Identifies poor quality documents and marks them for rescanning.
  • Ensures that all scanned documents are positioned correctly.
  • Identifies when it is appropriate to split and merge documents and performs the splits and merges correctly.
  • Appends and inserts pages/documents when and where appropriate.
  • Writes each indexed batch to the appropriate queue according to workflow procedure.
  • Maintains expected productivity and quality standards.
  • Assists in chart pick up and scanning depending on department need and by management direction.
  • Prepares all documents for scanning by removing staples, paper clips, sticky notes, and rubber bands.
  • Prepares all documents in date sequence from admission to date of discharge.
  • Groups like document types together, places in page order and in date order correctly 100% of the time.
  • Communicates any issues/concerns to the Team Leader or manager with clear detail.
  • Assembles all documents in appropriate order as outlined in chart prep policy and in correct order at least 100% of the time.
  • Ensures that all charts are assembled within the required time frame at least 100% of the time.
  • Maintains patient confidentiality by not revealing and or discussing patient information with anyone who does not have a legitimate interest in the information.
  • Willing to work days or shifts other than the one assigned in order to assist the department management with appropriate coverage of workflow.
  • Completes other duties as assigned by His/Her department manager.
