White Blossom Care Center is seeking an organized and detail-oriented, full-time Medical Records Assistant to join our healthcare team in San Jose, CA. Requirements

  • Minimum 1 year of office or clerical experience (experience in a medical office or skilled nursing facility is highly preferred).
  • Familiarity with medical terminology and electronic health record systems (e.g., PointClickCare).
  • Solid understanding of HIPAA privacy regulations and data security.
  • Strong organizational skills with a proven track record of accuracy in high-volume environments.
  • High School Diploma or equivalent required.

Responsibilities

  • Assemble, analyze, log, and file resident electronic and paper medical charts.
  • Review charts regularly to ensure completeness, accuracy, and compliance with state and federal regulations.
  • Process authorized requests for medical records from families, physicians, and legal entities under strict HIPAA guidelines.
  • Update electronic health records (EHR) accurately during resident admissions, transfers, and discharges.
  • Assist the Medical Records Director with daily office operations, scanning, and digital archiving.
