About The Position

This position is responsible for the accurate and timely completion of birth certificates and fetal death certificates. The Medical Records Analyst will gather necessary data, enter information into the VERI system, and ensure parental review and signatures. They will also educate parents on the birth certificate and social security number process, assist with establishing paternity, and provide necessary documents for insurance companies. Maintaining data integrity through report printing, verification with medical records and clinical staff, and making necessary corrections is crucial. The role involves managing on-site files, ensuring physician certification, and releasing birth certificates within a specified timeframe. Additionally, the analyst will manage unresolved records in the VERI queue and provide administrative support for the VERI system, including troubleshooting and assisting staff with any issues.

Responsibilities

  • Gather necessary data for the completion of Birth Certificate and Fetal Death Certificate forms.
  • Accurately enter all information needed for the birth certificate into the VERI system.
  • Obtain parental review and signatures on all information that will be included on the Birth Certificate, adhering to DOH guidelines.
  • Educate married parents on the process of their child's birth certificate and social security number.
  • Educate unmarried parents on legally establishing paternity and the process of submitting the Certificate of Parentage.
  • Provide parents with necessary documents, such as proof of birth for insurance companies.
  • Print basic reports (Daily Delivery Log, Admit Checks, DC Mom, DC Baby) to assure data integrity and organize workload.
  • Consult medical records and/or clinical resources (RN/MD) to verify data in the VERI and make needed corrections.
  • Print, scan, and email when alteration of electronic records is necessary for data integrity.
  • Verify data entry of all births and fetal deaths into the VERI.
  • Maintain a log of all births by date of birth and date of release to Morristown Town Hall.
  • Maintain on-site files of all birth certificate/fetal death applications.
  • Assure physician certification of each live birth for the Birth Certificate.
  • Complete all birth certificates no later than the day before the mother is discharged.
  • Communicate the reason for any exception to the timely completion standard to a colleague the next day.
  • Release Birth Certificates within 5 days of the birth.
  • Release all other release points once data integrity has been reviewed.
  • Maintain a low volume of unresolved records on the VERI queue.
  • Review unresolved fields each day to ensure records are not withheld from release points.
  • Assist L&D and Mother Baby staff when VERI problems occur.
  • Troubleshoot basic VERI computer problems or assist in obtaining help from ISS when necessary.
  • Attend assigned number of staff meetings for the year.
