About The Position

The Birth Certificate Clerk is responsible for coordinating and processing all birth registration documentation for newborns delivered within the hospital. This position ensures the accurate and timely completion of birth certificates, paternity acknowledgments, and related state-required forms while maintaining compliance with hospital policies, state regulations, and confidentiality standards. The Birth Certificate Clerk serves as a liaison between patients, clinical staff, state vital records agencies, and other departments to facilitate the birth registration process.

Requirements

  • High School Diploma required
  • Minimum 1 year experience in related role or administrative role preferably in a medical setting.

Nice To Haves

  • Experience working with birth registration systems or vital statistics preferred.
  • Hospital experience preferred.
  • Ability to work independently and manage multiple priorities.

Responsibilities

  • Interview mothers and families to obtain demographic information required for birth registration.
  • Complete and submit birth certificate information accurately and within mandated state timelines.
  • Review medical records to verify birth data, including infant demographics, delivery information, and parental information.
  • Assist parents with completing birth certificate worksheets and answer questions regarding the registration process.
  • Process Acknowledgments of Paternity (AOP) and other legal documentation as required.
  • Ensure all birth registration records are accurate, complete, and compliant with state and federal regulations.
  • Enter and maintain data within the Electronic Birth Registration System (VERI) and hospital information systems.
  • Monitor pending birth records and follow up on incomplete documentation.
  • Maintain confidentiality of patient information in accordance with HIPAA and hospital policies.
  • Prepare reports and maintain records related to birth registration activities.
  • Communicate with state vital statistics offices regarding corrections, updates, and reporting requirements.
  • Provide education to patients regarding obtaining certified birth certificates and other vital records.
  • Other relevant tasks as assigned.
