About The Position

The Medical Provider delivers high-quality, patient-centered medical care as part of an integrated, multidisciplinary team that partners with individuals experiencing homelessness and other barriers to health. Working both in clinic settings and at mobile and community-based sites, the provider is responsible for assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions using evidence-based, trauma-informed approaches. The provider participates in a shared telephone on-call rotation. Both part-time and full-time schedules are available.

Requirements

  • Graduate of an accredited school of medicine
  • Completion of medical residency in family medicine, internal medicine or internal medicine/pediatrics
  • Board certified or board eligible in family medicine, internal medicine or internal medicine and pediatrics
  • Possesses a current physician’s license in Maryland
  • Possesses CDS and DEA with DATA 2000 waiver licenses
  • BLS CPR certification
  • Minimum of 3 years of direct clinical care experience preferred (residency may satisfy this requirement for physicians)
  • Strong interpersonal and communication skills; able to build rapport with individuals with a wide range of personalities and communication styles. Demonstrated ability to collaborate within a multidisciplinary team.
  • Proficiency in electronic medical records.
  • Excellent clinical judgment, organization, and time-management skills.
  • Ability to remain flexible, adapt to changing environments and workflows and manage interruptions.
  • Cultural humility and sensitivity
  • Ability to engage compassionately with individuals who may be ill, distressed, or experiencing crises.
  • Commitment to self-assessment, integrating, giving and receiving constructive feedback

Nice To Haves

  • Experience in a community or public health setting working with underserved communities strongly preferred
  • Comfort working with individuals experiencing homelessness, behavioral health conditions, substance use disorders, and complex social needs.

Responsibilities

  • Provide comprehensive primary care including assessment, diagnosis, treatment, and referral across acute, chronic, and preventive domains.
  • Deliver patient-centered, trauma-informed, and nonjudgmental care tailored to individuals’ medical and social circumstances.
  • Provide care in multiple settings, including clinic locations, the mobile health unit, and community-based sites as needed.
  • Utilize evidence-based guidelines and clinical best practices to ensure high-quality care.
  • Participate actively on an integrated care team that includes medical, behavioral health, case management, and outreach staff.
  • Contribute to care planning, care team huddles, and multidisciplinary case conferences.
  • Coordinate care with internal and external partners to address social and structural determinants of health.
  • Incorporate up-to-date clinical information, research, and guidelines into practice.
  • Participate in peer reviews, supervisory and team meetings, required training, and quality improvement initiatives as required by the agency.
  • Maintain accurate, timely, and complete documentation and billing records in the electronic medical record.
  • Communicate clearly, compassionately, and efficiently with clients, colleagues, and partners.
  • Use sound clinical and professional judgment in managing time, workload, and competing priorities.
