About The Position

This position shall be responsible for assisting with the medical needs of people with intellectual and developmental disabilities in Individual Residential Alternatives (“IRAs”). The work schedule is full-time, 37.5 hours per week, primarily Monday - Friday weekdays, with flexibility across all shifts as required by the needs of the agency and the individuals we support. Travel is required and mileage is reimbursed. This position will be assigned to sites in Edwardsville, Canton, and Norfolk, though locations may be adjusted in the future.

Requirements

  • High School diploma or GED is required.
  • A minimum of one year of experience as a Direct Support Professional with Medication Certification is required.
  • Ability to pass all OPWDD-required background checks.
  • A NYS Driver’s License with two years of driving experience is required.
  • Acceptable motor vehicle abstract to meet Agency insurability standards.
  • Reliable personal vehicle for business use.
  • Effective verbal and written communication skills and knowledge in electronic health record usage.
  • Skilled in organizing, planning, exercising initiative, judgment, problem-solving, and maintaining effective relationships with providers, staff, individuals supported, and the public required.
  • Medium work; ability to lift and carry up to 50 lbs., lift and support individuals with and without help.
  • Ability to operate personal and Agency vehicles.
  • Ability to sit, walk, and stand for extended periods of time.
  • Some exposure to infectious diseases, hygiene issues, food, and other odors.
  • Ability to operate mechanical devices used by individuals.
  • Ability to tolerate extensive exposure to computer monitor/screen.
  • Ability to tolerate extensive use of computer keyboard.

Responsibilities

  • Responsible for routine review of required medical documentation and logs in the IRAs for accuracy and completion.
  • Visit all assigned sites at least once per week. Review the communication logs, appointment books, and minor incident tracking. Report all issues to the RN.
  • Review BM charts, MARs, and controlled counts for issues and communicate to the RN.
  • Complete monthly and weekly medication cupboard checks as directed by the RN. Ensure medication storage is appropriate and expired medications are disposed of and replaced.
  • Responsible for supporting the activities of the RN by gathering, organizing, and reporting information.
  • Attend team meetings at assigned sites. Ensure medical issues reported at the meetings are communicated to the RN.
  • Attend monthly nursing meetings to discuss and address concerns.
  • Compare MARs to orders before the beginning of the month and print copies for staff as directed by the RN.
  • Review charts as directed by the RN. Communicate to the RN when orders need to be updated, or appointments need to be made.
  • Schedule appointments as directed by the RN. Ensure appointments are written in the appointment books and that the House Manager is aware.
  • Communicate all appointments to the RN.
  • Prepare orders and send them to the providers as directed by the RN.
  • File orders, medication/consult referrals, and other forms as directed by the RN.
  • Responsible for providing care and support to the people we serve by coordinating and assisting with their medical needs.
  • Accompany individuals on medical appointments, locally and out of the area. Communicate concerns to the medical providers and ensure providers' orders and recommendations are communicated to the RN and IRA staff.
  • Implement medication changes as directed by the RN.
  • Pass medications as needed at any of the sites.
  • Work a flexible schedule as needed to cover medication times and appointments.
  • Responsible for additional duties as assigned by the Director of OPWDD Services or the RN.
