About The Position

Puzzle Healthcare is seeking an experienced Physiatrist (PM&R) to serve as Medical Director, providing clinical oversight and leadership across its post-acute and long-term care network. The role combines remote medical direction with periodic on-site support for complex rehabilitation procedures. The Medical Director serves as the clinical authority for PM&R across assigned regions, guiding care standards, documentation quality, compliance, and rehabilitation outcomes. Full-time and part-time opportunities are available, with a remote-first approach and meaningful hands-on clinical involvement.

Responsibilities

• Provide clinical leadership as the physician-of-record for PM&R services across assigned facilities.
• Conduct structured remote chart reviews, documentation oversight, and compliance audits.
• Review and co-sign clinical encounters per state and payer requirements.
• Establish and maintain physiatric care standards, documentation expectations, and clinical protocols.
• Offer remote guidance and feedback to therapy teams, APPs, and facility leadership.
• Identify trends in documentation, care quality, and training needs, and lead targeted improvement initiatives.
• Participate in regular virtual check-ins with clinical operations, compliance, and quality teams.
• Support onboarding of new clinicians and facilities.
• Stay current with PM&R guidelines, PDPM requirements, and post-acute regulatory updates.
• Provide occasional on-site support for complex procedures, advanced assessments, or facility needs.
