About The Position

We are seeking a compassionate and motivated Physician (MD/DO) to join our team in a dual-role position providing on-site primary care services within skilled nursing facilities while serving as Medical Director. This role includes fully funded participation in Certified Medical Director (CMD) training, offering an excellent pathway into clinical leadership. This opportunity is ideal for physicians seeking a flexible and predictable schedule while continuing their existing practice. Requirements

  • MD or DO degree required
  • Board Certified or Board Eligible in one of the following: Internal Medicine, Family Medicine, Geriatrics, Emergency Medicine
  • Active Kentucky medical license (or ability to obtain)
  • Interest in post-acute care, leadership development, and community-based medicine

Nice To Haves

  • CMD certification is not required to begin.

Responsibilities

  • Provide on-site primary care services to residents in skilled nursing facilities
  • Assess and manage acute and chronic medical conditions
  • Evaluate residents experiencing significant changes in condition
  • Collaborate with interdisciplinary care teams and facility leadership
  • Supervise and support NP/PA clinicians delivering routine and regulatory visits
  • Participate in quality improvement and patient care initiatives
  • Manage urgent clinical issues through telehealth coverage
  • Quality Assurance & Performance Improvement (QAPI)
  • Regulatory compliance and survey readiness
  • Interdisciplinary team leadership
  • Skilled nursing facility operations and best practices
