Howell, MI: Medical Director and Attending Physician, Full-Time
About The Position
Theoria Medical is seeking a Medical Director and Attending Physician to serve local skilled nursing facilities within a close-knit, mission-driven care community in Howell, MI. This role focuses on building relationships, providing consistent care, and making a lasting difference for vulnerable populations. The position offers a unique blend of medical excellence and technological advancement, serving the post-acute sector. Physicians will care for patients over time, building trust and strengthening continuity. The model offers flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team.
Requirements
- Physicians (MD/DO) must hold an active U.S. medical license and be board-certified or board-eligible in an appropriate specialty.
- New graduates who are board-eligible and actively pursuing licensing are encouraged to apply.
- Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

- Theoria Medical conducts criminal background checks and pre-employment drug testing on all candidates upon acceptance of contingent offer.
Responsibilities
- Serve as the attending physician for patients in each assigned SNF.
- Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
- Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
- Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
