Theoria Medical is seeking a Medical Director and Attending Physician to serve local skilled nursing facilities in Carmi, IL. Requirements

Physicians (MD/DO) must hold an active U.S. medical license and be board-certified or board-eligible in an appropriate specialty.
Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
Employee must be able to perform the essential functions of this position satisfactorily, with or without a reasonable accommodation.

Responsibilities

Serve as the attending physician for patients in each assigned SNF.
Collaborate with NPs/PAs to manage daily patient care and oversee clinical supervision.
Provide high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
Participate in Theoria’s Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
