About The Position

Weisman Children's Rehabilitation Hospital is seeking a Full-Time Medical Day Care Social Worker to join their skilled team of pediatric professionals in Northfield, NJ. This position is benefit eligible. Weisman Children's is a highly-acclaimed, community-based pediatric organization consisting of an acute care rehabilitation hospital, outpatient rehabilitation centers, and medical day care centers. They specialize in delivering individualized, family-centered care by including family and caregivers in every step of the rehab process, along with an interdisciplinary approach to treatment, to help patients reach their fullest potential. The Medical Day Care Social Worker (SW) provides comprehensive psychosocial assessment, care coordination, and family support services to pediatric patients and their families. The Social Worker is a key member of the Interdisciplinary Team (IDT) and is responsible for supporting the emotional, social, and financial well-being of children and families while ensuring compliance with facility policies and procedures, NJ Department of Health Pediatric Medical Day Care standards, interdisciplinary care planning requirements, and confidentiality and regulatory standards.

Requirements

• Licensed or certified by the New Jersey Board of Social Work Examiners
• Minimum one (1) year experience in pediatric or family social work
• Strong communication and interdisciplinary collaboration skills
• Computer proficiency (EMR, Microsoft Office, documentation systems)
• Knowledge of community resources and support systems
• CPR Certification (BLS)
