About The Position

The Medical Coordinator for La Jornada arranges and coordinates medical, dental, mental health, and pharmacy services for children in care, including urgent, emergent, and preventive care. The role maintains complete and confidential health records, communicates regularly with children’s care teams, coordinates appointments and interpretation services, and supports continuous quality improvement. The Medical Coordinator also helps create a positive medical environment for children, supports foster parents and staff with health-related needs, and assists program leadership with audits.

Requirements

Licensed Practical or Vocational Nurse (LPN/LVN) or higher health-related qualification, such as a Registered Nurse with a bachelor’s degree, or a bachelor’s degree in a relevant field.
Bilingual fluency in English and Spanish.
At least one year of employment in public health or a healthcare setting, with demonstrated experience in care coordination, health administration, or direct clinical care.
At least one year of experience working with children and/or adolescents in a child welfare or social service setting.
Valid driver’s license, excellent driving record, safe and clean personal vehicle, and current automobile insurance.
Ability to maintain compliance with required agency training programs.
Proficiency with desktop and laptop computers, smartphones, tablets, printers, fax machines, photocopiers, and word-processing, spreadsheet, and database software.
Willingness to work non-traditional hours and be available for on-call phone consultations as needed.
Ability to use manual dexterity and tactile, visual, and audio acuity; perform repetitive motions; sit and stand for prolonged periods; and sustain visual and mental attention.
Ability to occasionally lift up to 25 pounds, bend, pull, and carry.
Ability to travel frequently and drive for extended periods, with travel frequency varying according to program demands.

Responsibilities

Perform work consistent with NYAP’s mission, values, and philosophies, and provide children with a positive and appropriate medical environment.
Protect confidential health records and follow NYAP, state, and funder requirements for protected health information.
Work to obtain memoranda of understanding with medical, dental, and mental health providers.
Participate in child intakes and discharges, including administering COVID-19 tests, preparing medical documentation packages, and uploading documents to portals and health record systems.
Assess children’s medical, dental, and mental health needs and coordinate care in accordance with applicable policies and procedures.
Collaborate with children’s care teams and medical, dental, mental health, and pharmacy providers to coordinate appropriate care.
Assist healthcare providers with the PCU process to become approved providers.
Schedule initial, follow-up, wellness, and specialist appointments; prepare Treatment Authorization Requests and follow up on payment.
Transport children to and from medical appointments and accompany them during appointments; obtain medical clearances from healthcare providers.
Report medical emergencies through the SIR/CLE process and elevate medical concerns to supervisors, care teams, healthcare providers, and DHUC as appropriate.
Educate and support foster parents on medical policies and children’s medical care, assist with required responsibilities, and provide necessary authorization and identification documents.
Collect provider forms and health records, ensure they are complete, and upload them to appropriate portals and systems.
Complete and submit assigned documentation within required timeframes, including placement documents, assessments, medical packages, COVID-19 checklists, medication and temperature logs, medical summaries, and medical checklists.
Maintain accurate electronic and physical health records and securely file physical records in compliance with confidentiality requirements.
Communicate with children’s care teams and work toward quality improvement measures for medical records and upload timelines.
Keep medicine cabinets and first aid kits stocked with in-date over-the-counter medications, first aid supplies, rapid antigen COVID-19 tests, and other essential medical items.
Provide training and ongoing education to staff and foster parents on children’s health needs, medication administration, COVID-19 testing, CPR, and First Aid.
Maintain Red Cross CPR/First Aid Trainer certifications.
Assist and support program leadership during auditing processes.
Perform other duties as requested.
