About The Position

The Medical Coordinator is responsible for preparing consult sheets, reviewing consults, submitting medication record changes, attending medical and clinical team meetings, organizing the consent for treatment process, and preparing new monthly MARs. This role involves daily review of MARs and medication inventory to ensure accuracy and proper documentation of any medication errors. The Medical Coordinator also serves as a liaison between physicians, ancillary services, the individual/IDT team, and family, coordinating with consultants, dietary personnel, pharmacists, and labs as needed. The position requires a high degree of professionalism, respect for individuals, and adherence to privacy and attendance policies.

Requirements

  • High School diploma or General Education Degree (GED).
  • Minimum 2-year experience working with developmentally disabled population preferably OPWDD population with in a residential setting.
  • 1-2 years solid supervisory experience working with similar population.
  • AMAP, SCIP, and CPR certification required, but will consider applicants who agree to obtain certification within 90 days of position appointment.
  • Ability to physically restrain challenging behaviors that include aggression, self-injurious, and destructive behaviors.
  • Ability to apply proper lifting techniques.

Nice To Haves

  • Preferably relevant medical certifications and Associates degree.

Responsibilities

  • Prepare consult sheet for each appointment regardless of locale per agency guidelines.
  • Review all consults after appointment immediately upon return to facility.
  • Submit Literal Text changes, new or discontinued orders to ChemRx by the 15th of each month to ensure accurate medication record on next month’s MAR.
  • Meet weekly for Medical Meeting with RN and Manager to review outstanding tasks, diagnostic tests, and appointments to be completed.
  • Attend Clinical Team Meetings: Quarterly, Annual and semi-annual as scheduled and/or monthly clinical for ICF; Semi-annual, annual and as needed for IRA.
  • Organize Consent for treatment process, coordinating completion of documentation with RN, manager, MD.
  • Perform additional tasks related to consumer health care or medical system as deemed necessary by RN or Nurse Administrator.
  • Prepare new monthly MAR.
  • Daily review of MAR/ Medication inventory to ensure accuracy.
  • Ensure all medication errors are documented on an occurrence report and reported to the RN and manager.
