About The Position

This role involves a dual focus on medical coordination and direct support for individuals, ensuring their medical needs are met and assisting with daily living activities. The Medical Coordinator aspect involves collaborating with the RN and Residence Manager to manage medical appointments, documentation, medication administration, and supply management in compliance with OPWDD regulations. The Direct Support Professional aspect includes assisting individuals with their morning routines, meal preparation, laundry, and other daily needs as required.

Responsibilities

  • Schedule medical appointments in a timely manner and in accordance with regulations.
  • Prepare clinical consultation forms for all medical appointments.
  • Ensure sufficient staff are scheduled for medical appointments.
  • Accompany individuals and/or staff on medical appointments whenever possible.
  • Review and file all medical documentation after it has been signed by the RN and/or Residence Manager.
  • Ensure all medical appointments are up to date.
  • Ensure all individuals' lab work is up to date in collaboration with the RN.
  • Ensure all physician recommendations are documented and implemented.
  • Develop weekly/monthly medical appointment calendars.
  • Maintain AMAP certification.
  • Submit all prescriptions to ChemRx Pharmacy upon receipt.
  • Count and record medications upon delivery, ensuring accuracy if received by an AMAP.
  • Complete "Client-Specific Sheet" for new medication deliveries.
  • Check Medication Cart/Room weekly for expired medications and ensure a minimum of five (5) days' supply.
  • Reorder medications when the supply is insufficient for a minimum of five (5) days.
  • Ensure ChemRx Pharmacy receives all original prescriptions.
  • Review and log all control substances, performing weekly reviews for compliance and validation.
  • Discontinue medications as indicated by the prescribing physician in collaboration with the RN.
  • Ensure the Medication Room/Cart is clean, orderly, and sufficiently stocked with supplies.
  • Order medical supplies as needed.
  • Ensure all AMAP signatures are on the Medication Administration Record (MAR) prior to the first administration each month and file MAR documentation monthly.
  • Prepare off-site medications for home visits and trips, including necessary documentation.
  • Assist the RN in maintaining individuals' medical binders.
  • Assist the RN in completing various forms, including infection control, medical appointment, nursing report, and resident report.
  • Complete all paperwork as requested by the RN, Residence Manager, or Assistant Residence Manager.
  • Ensure quarterly pharmacy reviews are scheduled and completed.
  • Review and monitor Bowel Movement charts daily, reporting any issues to the RN and Manager.
  • Perform all other duties requested by the RN, Residence Manager/Assistant Residence Manager, Program Coordinator, or Program Director.
  • Assist with morning routines for individuals, including ADL outcomes, making beds, and breakfast clean-up/preparation.
  • Assist with dinner preparation if medical coordinator responsibilities are completed.
  • Assist individuals upon their return from programs, such as preparing lunch, tooth-brushing, and laundry.
  • Complete laundry if all medical coordinator responsibilities are completed.
  • Perform food shopping for dinner if needed and time is available.
