About The Position

Family Health Care Resources is a Community Clinic in East Los Angeles seeking a Bilingual (Fluent in Spanish) Licensed Social Worker to assist patients. This is a remote position, but candidates must reside in Los Angeles County. The role is full-time, Monday through Friday. Experience in Geriatrics is preferred due to the patient population being primarily elderly and requiring assistance with various social services and case management.

Requirements

Active Social Work License.
Must be bilingual in Spanish/English.
Competence in the physical, mental, and social needs of the elderly.
Must be able to legally work in the US.
Strong knowledge of Medicare, Medi-Cal, Social Security, Disability, Housing, Immigration, Covered California, and other local and state-level programs to assist the elderly and low-income patients.
MUST BE FLUENT IN SPANISH.

Nice To Haves

Geriatrics experience is preferred.

Responsibilities

Evaluate the medical and social needs of patients.
Provide initial and continuous assessment of each patient in the formation of an individual plan of care.
Serve as a liaison with the patient's home and family; and with other community entities that provide services to the patient.
Coordinate services to the participant to meet needs and avoid duplication.
Provide documentation for services provided and produce progress notes and periodic assessments of participant status.
Provide care coordination including, but not limited to, reading of mail, coordination of social benefits, and solicitation of resources.
Provide in-home assessment of participants.
