About The Position

BAMSI's HIV Services programs offer comprehensive, community-based support to individuals living with HIV/AIDS and those at risk of HIV, Hepatitis C, sexually transmitted infections, and substance use-related health concerns. Through case management, prevention services, outreach, education, and harm reduction strategies, we aim to help individuals access care, improve health outcomes, and achieve greater stability and wellness.

Requirements

  • Minimum of two (2) years of professional or volunteer experience in human services required
  • Case management or care coordination experience required
  • Phlebotomy skills preferred, or willingness to learn required
  • Valid driver's license and reliable transportation required
  • Ability to work occasional evening and weekend hours as needed
  • Proficiency with Microsoft Word, Teams, Outlook, and Electronic Health Records

Nice To Haves

  • Associate or Bachelor’s degree in a Human Services field preferred
  • Experience working directly with individuals living with HIV/AIDS
  • Experience supporting individuals with substance use disorders
  • Familiarity with harm reduction principles and practices
  • Experience working with underserved and vulnerable populations
  • Familiarity with the Brockton area preferred
  • Bilingual abilities in Haitian Creole, Cape Verdean Creole, Portuguese, Spanish, or other languages commonly spoken in the community strongly preferred
  • Individuals with lived experience, people of color, and LGBTQIA+ identified candidates are strongly encouraged to apply

Responsibilities

  • Conduct ongoing needs assessments and individualized care planning
  • Connect individuals to medical care, health insurance, specialty treatment, and behavioral health services
  • Assist clients with accessing social services, financial resources, and community-based supports
  • Provide transportation, interpretation, education, and advocacy services
  • Conduct home visits and transport clients to medical, legal, immigration, and social service appointments as needed
  • Develop and coordinate referrals based on identified client needs
  • Conduct outreach activities in collaboration with community organizations and partners
  • Identify and engage individuals living with HIV/AIDS who may benefit from services
  • Attend community meetings and events focused on health education, prevention, and access to care
  • Build and maintain strong working relationships with clients, providers, and community stakeholders
  • Collaborate with HIV Services staff on prevention, education, counseling, testing, and overdose prevention efforts
  • Screen, assess, and counsel individuals at risk of HIV, HCV, STI infection, or opioid overdose
  • Collect specimens through venous blood draw, finger stick, oral swab, and/or urine collection for testing services
  • Provide education regarding harm reduction, safer practices, HIV prevention, and treatment engagement
  • Maintain accurate documentation within CareWare and other Electronic Health Record systems
  • Complete charting, data collection, reporting, and follow-up activities in a timely manner
  • Assist with the operations of the BAMSI Community Food Pantry, including inventory management, ordering, and distribution
  • Attend trainings, conferences, and professional development activities to remain current on HIV, STIs, Hepatitis, substance use, and harm reduction best practices
  • Support program operations and perform additional duties as assigned
