About The Position

The Medical Case Manager is responsible for maintaining all resident (ECI Paths) medical records and ensuring compliance with standards set forth by licensing and regulatory bodies (DPW, Managed Care Organizations, & COA). This role coordinates all health care appointments, facilitates in-home visits, and tracks all visitation for Breinigsville residents.

Responsibilities

  • Maintains and updates the medical portion of each resident’s record, ensuring full compliance with all applicable regulations and required documentation standards.
  • Manages all client health records and medications within the Electronic Health Record (EHR) system, ensuring all services and supporting documentation are accurately uploaded and attached.
  • Ensures all emergency medical and contact information for residents is current and properly documented in the EHR.
  • Schedules, coordinates, and tracks all routine, specialist, and follow-up medical appointments to ensure compliance with licensing and regulatory requirements.
  • Collaborates closely with the Residential Coordinator and Lead Supervisor regarding staffing and medical appointment schedules; assists with appointment transportation as needed.
  • Obtains health insurance information in a timely manner for new residents when not provided at intake.
  • Schedules, coordinates, and facilitates all psychiatric appointments; supports medical providers by obtaining and recording residents’ height, weight, and blood pressure prior to medication management visits.
  • Orders, maintains, and monitors prescription medications to ensure availability of required doses for all residents.
  • Conducts and documents weekly medication counts.
  • Oversees medication administration, including training staff on mandated medication administration protocols.
  • Maintains certification as an approved medication-administration trainer.
  • Notifies parents and/or legal guardians of all medical appointments and any changes in medications, documenting all communication as a “medical correspondence” in the Credible system.
  • Participates in staff trainings and delivers trainings related to medical and emergency protocols.
  • Assists with staff training in CPR and first aid.
  • Facilitates quarterly in-home meetings with parents/families of Philadelphia County residents and maintains required documentation.
  • Provides support by periodically filling in for Child Care Workers and Supervisors as needed.
  • Performs additional duties as assigned.
