About The Position

Fourth Street Clinic is seeking a Medical Case Manager (MVP Program) to provide patient-centered case management for unhoused and medically vulnerable individuals. The Medical Case Manager is a key member of the Medically Vulnerable Population (MVP) team. This role ensures continuity of care, addresses social determinants of health, and advocates for patients to achieve optimal health outcomes. Requirements

  • High School Diploma or equivalent
  • Strong attention to detail and commitment to accuracy in all work
  • Ability to work collaboratively and demonstrate a caring, compassionate approach to patient care

Nice To Haves

  • Bachelor's degree in a healthcare-related field
  • 2+ years of experience in medical case management or related patient support functions, specifically in ambulatory care
  • Bilingual in English and Spanish
  • Experience working with medically vulnerable or unhoused populations

Responsibilities

  • Provide medical case management for a designated caseload of 25–34 patients
  • Conduct thorough needs assessments for medically vulnerable patients
  • Develop and update individualized, patient-centered care plans
  • Coordinate and facilitate delivery of prescribed medical and supportive services
  • Arrange transportation for patients to access services both within the clinic and with community partners
  • Collaborate with interdisciplinary teams and external service providers to ensure continuity of care
  • Maintain accurate patient documentation and meet medical/data reporting requirements
  • Stay current on clinic policies, procedures, systems, and electronic health records
  • Participate in weekly check-ins with the MVP Clinic Manager
