Under the supervision of the Case Management Supervisor, the Medical Case Manager (MCM) - Housing is responsible for supporting team members and delivering services that lead to satisfactory health outcomes for people living with HIV/AIDS in Affinity Health Center’s (AHC) service area. This role involves researching and maintaining a list of housing programs, developing and maintaining relationships with landlords and partners, and providing referrals when appropriate. The MCM will also provide information to clients, other MCMs, and partners on housing resources, eligibility, and budgeting. A key part of the role is providing direction to the MCM team on patient eligibility, ensuring documentation is compliant and correct in patient charts, and researching landlord/tenant law. The position also assists with chart audits, reviews data entry in EMR for accuracy and compliance, and works with the Data Quality Team to run reports and review metrics related to housing. The MCM will write monthly and/or quarterly reports for funders and partners, and collaborate with the Accounting Team to ensure documentation and subsidy payments are correct. Direct communication with patients and MCM staff is required to obtain necessary documentation and ensure patient charts are up-to-date and accurate. The Medical Case Management duties include managing a caseload of up to 50 patients per year, conducting comprehensive assessments of patient needs, goals, and eligibility for state and federal programs like ADAP and HMAP. The role involves building relationships with patients, developing individualized action plans, advocating for services, and coordinating with internal/external providers. Support is provided for patients completing applications for various state and federal programs, and education is given on eligibility requirements and insurance coverage. Ongoing evaluation and support are provided to ensure patient retention in medical care, with semi-annual and annual reassessments. Documentation of intakes, data, and service plans is done using electronic health records and databases like Provide Enterprise, ensuring services align with Ryan White guidelines. Participation in Continuous Quality Improvement (CQI) activities is also expected.
