About The Position

Get out of the clinic routine and deliver preventive care directly in patients’ homes across Washington. This field-based role involves visiting patients in their homes to complete Medicare Annual Wellness Visits and Well-Child Visits, focusing on prevention and early intervention. You will build stronger patient relationships without rushed visits, utilize modern EMR tools for charting, and work independently with clinical team support.

Requirements

• 1+ year as a Medical Assistant
• CMA, RMA, or equivalent certification
• 6+ months in primary care or internal medicine
• Comfortable with state travel
• Reliable vehicle and valid driver’s license
• Strong communication and documentation skills

Nice To Haves

• Medicare Annual Wellness Visit experience
• Well-Child Visit experience
• Home health or community-based care experience
