About The Position

Get out of the clinic routine and deliver preventive care directly in patients’ homes across Michigan. This field-based role involves state travel and focuses on delivering preventive care directly to patients in their homes. You will complete Medicare Annual Wellness Visits and Well-Child Visits, focusing on prevention and early intervention. The position allows for building stronger patient relationships without rushed visits, utilizing modern EMR tools, and working independently with clinical team support.

Requirements

• 1+ year as a Medical Assistant
• CMA, RMA, or equivalent certification
• 6+ months in primary care or internal medicine
• Comfortable with state travel
• Reliable vehicle and valid driver’s license
• Strong communication and documentation skills

Nice To Haves

• Medicare Annual Wellness Visit experience

• Home health or community-based care experience
