About The Position

Get out of the clinic routine and deliver preventive care directly in patients’ homes throughout Maricopa County while supporting patients across Arizona and other markets as needed. This field-based role involves visiting patients in their homes to complete Medicare Annual Wellness Visits and Well-Child Visits, focusing on prevention and early intervention. You will build stronger patient relationships without rushed visits, chart using modern EMR tools, and travel to support patients across Arizona and other regions. The role requires working independently with clinical team support.

Requirements

• 1+ year as a Medical Assistant
• CMA, RMA, or equivalent certification
• 6+ months in primary care or internal medicine
• Comfortable with regional and overnight travel
• Reliable vehicle and valid driver’s license
• Strong communication and documentation skills

Nice To Haves

• Home health or community-based care experience
