The Quality Performance Improvement, Population Health Team is now hiring for a compassionate and adaptable Medical Assistant! The Population Health MA (PHMA) role focuses on Quality Performance Improvement within the Population Health area. This position will work fully in-person at our Shared Services Building in West Boise. This is primarily a phone position focused on closing gaps in care by performing proactive outreach with patients and their care. This role will support patient care coordination for 180 independent affiliate clinics (network partners) and Saint Alphonsus Medical Group family medicine clinics. The position requires self-management, critical thinking skills, ability to analyze data sets, ability to provide gap in care education to clinic staff and providers, accountability for all workflows, team collaboration, and willingness to be flexible day to day. The Saint Alphonsus Health Alliance (Alliance) is a physician-led network sponsored by Saint Alphonsus Health System. The Alliance represents a new model of healthcare delivery, known as a Clinically Integrated Network. Our Clinically Integrated Network is based on a strategy to tightly align physicians, hospitals and payers to provide better access to care, better clinical quality and control costs. Requirements

  • 3 years of previous MA clinical experience required; 5 years preferred.
  • High School Diploma or equivalent required.
  • MA certification required within 90 days of eligibility to take a national certification exam from one of the following Saint Alphonsus approved agencies: American Association of Medical Assistants (CMA), American Medical Technologists (RMA), National Healthcare Association (CCMA), National Center for Competency Testing (NCMA), National Association of Health Professionals (NAHP), American Medical Certification Association (CMAC), National Phlebotomy Certification Examination (NPCE MA).
  • Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire.
  • Electronic health record (EHR) experience required.
  • Proficient in EPIC and Excel.
  • Must possess a curious mindset for facilitating and leading change.
  • Payer/insurance experience.
  • Professionalism and work at the top of licensure.

Nice To Haves

  • Experience with process improvement or Lean philosophy preferred.

Responsibilities

  • Primarily contacting patients over the phone focused on closing gaps in care by performing proactive outreach with patients and their care.
  • Supporting system-wide quality performance goals for HEDIS and STAR measures.
  • Assisting patients with scheduling health screenings, patient follow up with fit kits, Cologuard tests, etc., if they completed requested testing, and next steps with care after receiving results from their primary care provider.
  • Analyzing preventative health care screenings, working on gaps in care lists, pre-visit planning for providers, reviewing payor lists for open gaps in care, completing chart review for documentation of gap closure ensuring accurate and appropriate documentation of patients’ clinical complexity.
  • Providing education, training, support, and collaboration with primary care clinical teams and providers to support Quality Performance Improvement initiatives (Gaps In Care Closure) focused on Quality measures (HEDIS).
  • Adding value to continually improve internal processes for optimal provider and patient experience and improve patients' health outcomes and quality of life.
