About The Position

This role supports the High-Risk Prioritization (HRP) and Transitions of Care (TOC) programs, which are critical to improving patient outcomes, reducing avoidable utilization, and ensuring regulatory compliance. The Medical Assistant will conduct outreach and care coordination activities for a panel of over 30,000 empaneled patients, supporting the operational, clinical, and quality performance of all Colorado-employed Internal Medicine and Family Practice (IMFP) clinics. This position manages a substantial and complex monthly workload, including post-hospital discharge follow-up calls and development of individualized patient profile sheets for Primary Care Providers (PCPs). The role supports provider productivity, patient satisfaction, quality gap closure initiatives, revenue generation, and achievement of organizational performance measures, including HEDIS and Medicare Advantage quality goals. The Medical Assistant will identify and address care gaps to improve patient outcomes, enhance the patient experience, support National Patient Satisfaction (NPS) objectives, and contribute to achievement of 5-Star quality ratings. This role also supports organizational growth and Medicare Advantage initiatives through timely patient engagement, care coordination, and effective management of transitions of care. The Medical Assistant serves as the patient's first point of contact within the clinic, demonstrating C-I-CARE principles while providing exceptional customer service. Responsibilities include performing patient check-in and check-out functions, verifying insurance eligibility and benefits, answering and routing incoming calls, scheduling appointments, and collecting patient copays and outstanding balances. The position offers the flexibility to work remotely from anywhere within the U.S. The schedule is Monday-Friday, 8 am-5 pm Mountain Time.

Requirements

  • Completion of a Medical Assistant program
  • Basic Life Support (BLS) certification from AHA or willing to obtain by date of hire
  • 2+ years of experience working as a Medical Assistant

Responsibilities
