Novocardia, a division of CVAUSA, is seeking a talented, goal-oriented, and proactive Medical Assistant to play a key role in supporting the medical team and patients. This role is crucial for the Heart Failure Program and other value-based care management programs, focusing on patient enrollment, engagement, coordination, and providing excellent patient-centric care. The Medical Assistant will manage patient cohorts, coordinate care with remote teams, initiate patient intake, and ensure patient needs are met, including medication reconciliation and coordinating tests/referrals. The position requires maintaining patient records, following refill procedures, collecting insurance information, and organizing patient education. The role involves remote management of patients across different practices and states, requiring strong organizational skills and the ability to handle urgent situations. Clerical duties, basic triage, and participation in interdisciplinary team meetings are also part of the responsibilities.
