The primary purpose of this role is to provide safe, quality patient care while following all standards of care, regulatory requirements, and provider orders. This includes timely and accurate documentation to assist in billing of services to ensure a complete medical record, and providing patient and family education. The role also ensures patient satisfaction by assisting in the identification and implementation of strategic initiatives in accordance with Cass Regional Medical Center’s Mission, Vision and Values, utilizing principles of AIDET and managing up.

Requirements

  • Current Medical Assistant or LPN certification.
  • BLS required.
  • Knowledge of nursing skills; patient triage, pharmaceuticals, operation of equipment and instruments, insurance company requirements, disease processes and regulatory requirements.
  • Organizational and problem-solving skills
  • Effective professional and interpersonal communication skills
  • Ability to multi-task and prioritize patient assessment and care
  • Ability to act independently.
  • Use of critical thinking skills to identify, analyze and prioritize issues.
  • Assist in identification of solutions in collaboration with others.

Responsibilities

  • Assists in implementation of plan of care for all assigned patients daily (50% of time)
  • Sets up appropriate equipment and/or instruments for procedures as directed by provider.
  • Cleans exam and procedure rooms between patients and at the end of each clinic day.
  • Reviews and updates patient history and medications.
  • Obtains test results and other documentation.
  • Identifies the patient using the two approved patient identifiers prior to any patient care, including medication administration and specimen collections.
  • Accompany provider upon patient interview and examination.
  • Utilizing AIDET, introduce self to patient and explain role.
  • Documents vital signs to include weight, height, blood pressure, pulse, and temperature at each visit.
  • Documents pain level and other required assessments/regulatory requirements at each visit including medication reconciliation.
  • Documents and reports all critical vital signs and critical test results to the ordering provider per hospital policy and implements further orders immediately.
  • Documents details in the patient record.
  • Maintains patient care documentation logs as assigned per clinic/provider and regulatory agencies.
  • Assists providers with documentation assistance as applicable.
  • Notify the provider of any EHR alerts. Alerts must be addressed by the provider.
  • Assists the provider in navigating the EHR, responding to messages as directed by the provider, locating information for review (previous notes, reports, test results).
  • Ensure accuracy and completion of the medical record in collaboration with the provider; check medical record for inconsistencies, notify provider and check to ensure the inconsistencies are resolved.
  • Performs ordered testing as required in the clinic; completes and documents required quality controls.
  • Administers and documents medications.
  • Reviews discharge instructions with patients.
  • Completes documentation of referrals and pre-certs in a timely manner to maintain continuity of care.
  • Schedules patient appointments utilizing provider template.
  • Assists in obtaining specimen collections at the request of the provider.
  • Authorizes prescription refills, completes dressing changes/removal and suture removal at provider's request.
  • Actively participates in clinic operations (20% of time)
  • Orders supply and maintains appropriate levels of inventory for effective patient care.
  • Responsible for care (cleaning and storage) of equipment and instruments prior to, during and after procedure, including appropriate transfer to the decontamination area.
  • Follows all Manufacturer Instructions for Use (IFU) for pre-cleaning and cleaning instruments and equipment and High-Level Disinfection (HLD) as appropriate.
  • Participates in process improvement activities.
  • Reports all patient care problems/equipment issues to manager immediately.
  • Tags and removes malfunctioning equipment and completes work requisition.
  • Completes monthly quality and safety checks as directed, checking for expired medications and supplies.
  • Assists in training of new employees.
  • Assists provider with all procedures and carries out orders (10% of time)
  • Appropriately prepares rooms prior to each patient visit, obtaining equipment and instruments.
  • Explains procedure to each patient prior to the procedure.
  • Obtains consent or ABN if required prior to the procedure.
  • All provider orders are carried out and appropriately documented in the medical record.
  • Medications are administered per policy and provider order.
  • For each medication administered, documents in the medical record: medication, dose, site, route, time and patient adverse reaction, if present.
  • Effectively communicates discharge instructions to patient and responsible party at the time of discharge.
  • Completes referrals and pre-authorizations as needed.
  • Scans documents into the medical record as directed.
  • Maintains confidentiality and privacy of information in accordance with HIPAA regulations and organizational policies.
  • Manages telephone calls, faxes, and messages (10% of time)
  • All telephone calls are returned in a professional manner and documented by the end of the shift.
  • All patient phone calls are documented in the medical record.
  • All telephone calls, faxes, and messages are carried out per provider's instructions and documented in the medical record.
