For more than 55 years, Family Health Centers of San Diego’s (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons. FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country, operating more than 90 sites across San Diego County, including primary care clinics, behavioral health facilities, physical rehabilitation clinics, dental clinics, vision clinics, outpatient substance use treatment programs, mobile medical units, mobile counseling centers, urgent care centers, and a pharmacy. The staff provides care to over 227,000 patients each year, with a significant portion being low-income and uninsured. FHCSD offers a wide range of services including adult care, chronic disease management, pediatrics, comprehensive women’s care, dental, vision, case management, physical rehabilitation, speech therapy, vaccinations, infectious diseases, behavioral health, substance use counseling, and various specialty services. The organization also provides supportive services to the unsheltered. The Medical Assistant in the Nursing Information Center performs telephone patient engagement, assesses patient understanding of care, assists with transitional care activities, collaborates on population health management functions, schedules appointments, educates patients on follow-up care and the importance of a Primary Care Physician (PCP), and reinforces patient/family education.
