About The Position

For more than 55 years, Family Health Centers of San Diego’s (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons. FHCSD is one of the top 10 largest federally qualified health centers (FQHCs) in the country, operating more than 90 sites across San Diego County, including primary care clinics, behavioral health facilities, physical rehabilitation clinics, dental clinics, vision clinics, outpatient substance use treatment programs, mobile medical units, mobile counseling centers, urgent care centers, and a pharmacy. The staff provides care to over 227,000 patients each year, with a significant portion being low-income and uninsured. FHCSD offers a wide range of services including adult care, chronic disease management, pediatrics, comprehensive women’s care, dental, vision, case management, physical rehabilitation, speech therapy, vaccinations, infectious diseases, behavioral health, substance use counseling, and various specialty services. The organization also provides supportive services to the unsheltered. The Medical Assistant in the Nursing Information Center performs telephone patient engagement, assesses patient understanding of care, assists with transitional care activities, collaborates on population health management functions, schedules appointments, educates patients on follow-up care and the importance of a Primary Care Physician (PCP), and reinforces patient/family education.

Requirements

  • Medical Assistant diploma from an approved course.
  • High School Graduate (or GED equivalent) or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.
  • Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, or Advanced Trauma Life Support, or Advance Resuscitation Training.
  • Valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law (limits of $100,000 are recommended).
  • An acceptable driving record.
  • Ability to display cultural sensitivity and respect to ethnically and culturally diverse patient populations.

Nice To Haves

  • At least 1 year of experience in ambulatory care setting or similar setting.
  • Bilingual in English/Spanish.

Responsibilities

  • Perform telephone patient engagement as assigned by the manager using clear protocol on all calls that are transferred.
  • Assess level of patients’ understanding of care and assist in navigating FHCSD programs and services.
  • Assist with transitional care activities for FHCSD patients.
  • Collaborate with team members in supporting the population health management functions and interventions.
  • Schedule appointments for patients who require more than basic information.
  • Educate patients on the need for follow-up care.
  • Establish Primary Care Physician (PCP), and educate on the importance of continuity of care in using PCP.
  • Reinforce patient/family education provided to FHCSD patients.
  • Attend staff meetings and in-services or complete make-up assignments.
  • Follow FHCSD policies and procedures to ensure positive patient outcomes.
  • Perform other duties as assigned.
