About The Position

This full-time Medical Assistant position in Chronic Disease Management at Advocate Sherman Hospital's Health Management Resources (HMR) department focuses on providing clinical support to achieve health outcomes and integration goals. The role involves preparing medical records, obtaining and documenting accurate patient data, assisting physicians with registries, and communicating necessary information for clinical integration. The Medical Assistant will manage patient flow, prepare rooms and patients, perform ordered tests and procedures, maintain tracking logs, and identify/communicate abnormal test results. Responsibilities also include accurate specimen collection, maintaining quality control logs for CLIA certification, administering medications as ordered, and communicating with pharmacies for refills. The role requires anticipating needs and completing all necessary documentation, distributing patient education, and assisting patients with obtaining ordered services as part of discharge planning. Communication with other providers for continuity of care and documenting all patient interactions in the medical record are essential. The position also emphasizes patient satisfaction, creating a patient-centered environment, and demonstrating professional image and Advocate Behaviors of Excellence. Adherence to Health Information Management (HIM) and HIPAA standards is critical, including maintaining complete and accurate medical records, participating in audits, ensuring confidentiality, and obtaining release of information in accordance with policies. Compliance with infection control, OSHA safety standards, and accrediting organization regulations is required, along with maintaining a safe work area and promoting a culture of safety by reporting safety events and near misses. The role also involves contributing to appropriate charge capture, collaborating on coding and charges, maintaining inventory, understanding revenue and expense management, and promoting a green environment. Miscellaneous duties include maintaining knowledge of current standards of care, participating in continuing education, completing annual competency reviews, and performing other assigned duties.

Requirements

  • High School Diploma or GED
  • Graduate of a medical assistant program or at least one year of medical assistant or equivalent experience.
  • Good communications skills.
  • Experience using a keyboard and computers.
  • Familiarity with equipment used in ambulatory care.
  • Current AHA CPR certification.
  • Current Medical Assistant Certification/Registration required.
  • Non-certified MAs must obtain certification or registration within one year of hire with AMG approved certifying bodies.
  • Ability to travel locally to multiple sites as needed.

Responsibilities

  • Provides support in the delivery of quality clinical care
  • Supports achieving AMG Clinical Integration goals by medical record preparation, i.e. assures appropriate flow sheets and charts are available at time of patient visit.
  • Accurate data is obtained and documented.
  • Assists the physician in maintaining registries.
  • Communicates to provider any outstanding data, tests or measures required for Clinical Integration.
  • Prepares for patient visit by performing chart preparation.
  • Manages patient flow, including preparing rooms, patients in accordance with Advocate Medical Group (AMG) policies and procedures and rooming standards.
  • Performs tests/procedures as ordered.
  • Maintains test tracking log, communicating test results per policy.
  • Identifies and communicates abnormal or delayed test results to provider.
  • Responsible for accurate specimen collection including patient identification and labeling based on laboratory collection procedures.
  • Accurately performs and maintains practice site quality control logs in accordance with standards for maintaining CLIA certification.
  • Administers medications as ordered by provider including documentation.
  • Communicates with pharmacies to refill medication per approved protocol.
  • Anticipates the need for and completes all documentation necessary to ensure quality care including chief complaint, vital signs, immunizations/medications, history, procedures and messages.
  • Distributes patient education and discharge instructions per provider direction.
  • Direct patient questions regarding education to provider.
  • Assists patients in obtaining additional services ordered by the physician prior to leaving the office as part of discharge planning.
  • Communicates with other providers to ensure continuity of care.
  • Documents all patient communication in the medical record.
  • Provides efficient, high-quality service to patients and families during appointments/visits, telephone call.
  • Demonstrates respect, empathy and supports patient and family's cultural and spiritual beliefs.
  • Participates in activities to evaluate and improve processes that will contribute to patient satisfaction, associate and provider engagement. satisfaction.
  • Creates and maintains an environment that is patient centered as evidenced by availability of same day appointments, future appointments and referrals made.
  • Responsive to the needs and concern of patients, associates and providers and works closely to resolve issues with involved parties.

  • A complete and accurate medical record will be maintained for every patient who is evaluated or treated within the medical group.
  • Participates in medical record audits and indicator development
  • Maintains confidentiality in all aspects of patient care and in communication in person and via the telephone.
  • Assures that the medical record and all relevant diagnostic reports are available at the start of the visit.
  • Obtains medical record release of information in accordance with HIPAA policy and Advocate/AMG policies and procedures.
  • Disposes of papers containing patient identifiable information according to policy.
  • Maintains medical record filing by scanning or re-filing of charts.
  • Utilizes Advocate/AMG forms.
  • Complies with established infection control, OSHA safety standards and accrediting organization regulations.
  • Maintains work area in accordance with the Advocate/AMG Operating System standards.
  • Participates in safety drills.
  • Locates emergency equipment during drill as appropriate.
  • Demonstrates culture of safety behaviors/techniques.
  • Promotes culture of safety by identifying threats to patient, family and associate safety.
  • Reports safety events and near misses in a timely manner and completes using appropriate reporting methods.
  • Assists in the implementation of corrective actions.
  • Demonstrates order completion and documentation for appropriate charge capture and risk reduction.
  • Collaborates with provider and billing department to accurately code and capture charges.
  • Maintains inventory par levels using standard products, AMG AOS standards.
  • Understands revenue and expense management targets and is engaged in achieving same.
  • Promotes a green environment by adhering to organizational guidelines for recycling and prudent utilization of resources.

  • Maintains knowledge of current standards of care.
