About The Position
New York Cancer & Blood Specialists (NYCBS) is seeking a dedicated CNA/Medical Assistant to join their Chronic Care Management team. This role involves introducing the CCM program to eligible patients, obtaining consent, and documenting it in the EMR. The position requires reviewing and monitoring patient care plans, advocating for patients, collaborating with wellness services, and facilitating communication between providers and patients. All activities must be documented in the EMR system.
Requirements
- High School Diploma or equivalent.
- Certified CNA, or Medical Assistant.
- 2+ years of applicable experience.
- Proficiency in Word/Excel.
Nice To Haves
- Bilingual, English/Spanish a plus.
Responsibilities
- Introduce the CCM program to eligible patients via telephone.
- Obtain verbal consent and provide appropriate documentation in the EMR system.
- Review and monitor documents related to the patient’s individualized care plan under direct supervision of the CCM Supervisor.
- Advocate for patients.
- Collaborate with other wellness services at NYCBS.
- Provide educational information.
- Facilitate the transmission of information between providers and patients, as well as access to services needed.
- Document all activities in the EMR system.
