About The Position

EPIC Health is seeking a compassionate, bilingual Mandarin-speaking Medical Assistant Care Manager to join our growing Care Management team in Houston, Texas. This role is ideal for someone who enjoys building relationships with patients, coordinating care, improving health outcomes, and serving as an advocate throughout the healthcare journey. As a Care Manager, you will work closely with primary care providers and an interdisciplinary healthcare team to support patients with chronic conditions, coordinate specialty care, improve medication adherence, address barriers to care, and help patients achieve their health goals. Requirements

  • Fluent in Mandarin and English (spoken and written).
  • High School Diploma or GED.
  • Experience working in a healthcare setting.
  • Excellent communication and interpersonal skills.
  • Strong organizational and documentation abilities.
  • Comfortable using electronic medical records and Microsoft Office applications.

  • Medical Assistant certification or equivalent experience
  • Community Health Worker (CHW)
  • Licensed Vocational Nurse (LVN)
  • Care Management or Care Coordination experience
  • Population Health or Chronic Care Management experience
  • Experience working with Medicare, Medicaid, or value-based care populations
  • Behavioral Health or Case Management experience

Responsibilities

  • Serve as the primary point of contact for Mandarin-speaking patients enrolled in care management programs.
  • Conduct patient outreach by phone and in person.
  • Educate patients on chronic disease management, medications, preventive care, and healthy lifestyle choices.
  • Coordinate appointments, referrals, diagnostic testing, and follow-up care.
  • Collaborate with physicians, advanced practice providers, behavioral health professionals, pharmacists, and specialists.
  • Identify social determinants of health and connect patients with appropriate community resources.
  • Document all patient interactions accurately within the electronic medical record.
  • Monitor patient progress and escalate clinical concerns to licensed healthcare professionals when appropriate.
  • Support quality initiatives and value-based care programs.
  • Maintain patient confidentiality and comply with HIPAA regulations.
