This team member will be part of our centralized Ambulatory Care Management department alongside Care Coordinators/Care Managers, MSWs and a RD. They will contact patients after emergency room visits to provide transitions of care support as well as manage incoming patient calls from patients after their hospital discharge. The team member will manage referrals to our care management team (RNs, RD, MSWs) and provide support for scheduling, rescheduling, cancelations and no shows. The team member will complete benefit verifications for our statewide team for recently discharged patients to support care management workflows. The first six weeks would be a combination of work from home and in office time at either 60th St. or Corewell Health Place on Monroe. Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.
