About The Position
The Lodge at Jasper is seeking a Med Tech / Certified Medication Aide. The Med Tech monitors residents’ health and well-being and provides leadership support and assistance with resident care and services. The role includes medication care assistance and administration, communication with resident support services and families, and maintaining clinical quality assurance while supporting the wellness elements of Phoenix programs and services. The position supports resident assessments and Personalized Service Plans (PSPs), documentation and follow-through when residents’ clinical care needs change, monthly wellness visits, and monthly collection of residents’ weights and vital signs. The role also assists with resident care as needed and reports changes in medication or service levels to leadership.
Requirements
- Must be a certified Medication Aide and/or meet state requirements for a medication aide.
- Must demonstrate leadership capabilities.
- Ability to work weekends, evenings, and flexible hours and be available during peak service delivery days and times.
Nice To Haves
- A minimum of one year of experience working with elders or disabled individuals is preferred.
- Knowledge and experience in assisted living, dementia care, or skilled nursing are preferred.
Responsibilities
- Identify residents’ ongoing needs and services through the assessment and Personalized Service Plan (PSP) process.
- Ensure appropriate follow-through and documentation when a resident’s clinical care needs change.
- Support the Wellness Nurse in completing clinical sections of monthly wellness visits and PSPs so they accurately reflect residents.
- Inform the Wellness Director and/or Executive Director of medication or service-level changes that may result in a higher tier level or potential move-out.
- Assist with obtaining each resident’s monthly weight and vital signs before the monthly wellness visit is completed.
- Assist with resident care as needed.
- Provide medication care assistance and administration.
- Communicate with resident support services and families.
- Maintain clinical quality assurance and support wellness program elements.
