About The Position

The MDS (Minimum Data Set) Specialist plays a critical role in ensuring the accuracy and compliance of patient assessment data within long-term care facilities. This position is responsible for coordinating and completing comprehensive assessments that inform care planning and regulatory reporting. The MDS Specialist collaborates closely with interdisciplinary teams to analyze patient data, identify care needs, and support quality improvement initiatives. By maintaining up-to-date knowledge of federal and state regulations, the specialist ensures that all documentation meets required standards and supports reimbursement processes. Requirements

  • Licensed Vocational Nurse or Registered Nurse (RN) license in the United States.
  • Minimum of 2 years experience in long-term care or skilled nursing facility settings.
  • Demonstrated knowledge of MDS 3.0 assessment protocols and CMS regulations.
  • Proficiency with electronic health record (EHR) systems and MDS software.
  • Strong attention to detail and excellent organizational skills.

Nice To Haves

  • Certification as an MDS Coordinator or related credential.
  • Experience with quality measures and reimbursement methodologies such as PDPM.
  • Familiarity with interdisciplinary care planning and clinical documentation improvement.
  • Advanced training in gerontology or long-term care management.
  • Experience working with diverse patient populations and multidisciplinary teams.

Responsibilities

  • Conduct comprehensive MDS assessments for residents in accordance with federal and state guidelines.
  • Collaborate with nursing, therapy, and medical staff to gather accurate clinical information for care planning.
  • Review and validate assessment data to ensure accuracy and compliance with regulatory requirements.
  • Prepare and submit timely reports to regulatory agencies and assist with audits and surveys.
  • Provide training and support to clinical staff on MDS processes and documentation standards.
  • Monitor changes in resident conditions and update assessments accordingly to reflect current care needs.
  • Utilize MDS data to identify trends and support quality improvement initiatives within the facility.
